Extended Leave Application Form
Parents and carers who would like to volunteer at our school for any reason (e.g. canteen, class helpers, reading groups, athletics carnivals, special events, etc.) are required to complete a Digital Volunteer Declaration.
Prior to commencing volunteer work, please contact the office to provide your full name, email address and mobile number. The Department of Education will then send an invitation via email to you to complete an online Digital Declaration. Next time you attend the school, you will need to present a photo ID to finalise the screening process.
